0 Quick Apply 11 hours ago Full Job Description Paramount Skilled Nursing and Rehabilitation at Chambersburg Road is now hiring an Registered Nurse Assessment Coordinator (RNAC) Paramount Nursing and Rehabilitation is a 92 bed, skilled nursing facility located on a beautiful 30-acre senior living campus in Central Pennsylvania.
We are located in Fayetteville, PA, halfway between Chambersburg and Gettysburg in Route 30.
The RNAC assists the Director of Nursing with ensuring that documentation in the center meets Federal, State, and Certification guidelines.
The RNAC coordinates the RAI process assuring the timeliness, and completeness of the MDS, CAAs, and Interdisciplinary Care Plan.
The primary purpose of the job position is to conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of Pennsylvania and the policies and goals of this facility.

Responsibilities:
Conduct or coordinate the interviewing of each resident for the resident's assessment Evaluate each resident's condition and pertinent medical data to determine the need for special assessment activities or need to amend the admission assessment Develop and implement procedure with the DON to inform all assessment team members of the arrival of newly admitted resident Assist with the care plan portion of the resident's discharge plan Ensure that a complete resident assessment is conducted within fourteen days of a significant change in the resident's condition Coordinate the review and revision to the resident's care plan by the interdisciplinary team after each quarterly review or other assessment, assuring that the care plan is evaluated and revised each time an assessment is done or when there is a change in the resident's status Ensure that each portion of the assessment is signed and dated by the person completing the portion of the MDS Responsible for transmission of resident information to the state per requirements Perform other duties as assigned Education and
Qualifications:
Experience in a skilled nursing facility Previous experience dealing with insurance assessments required.
Candidate must have an understanding/ knowledge base of MDS/ reimbursement Candidate must be fluent with care plans and the care plan process Graduate of an accredited school of nursing.
Must possess a current, unencumbered, active license to practice as a Registered Nurse (RN) in Pennsylvania.
Previous long-term care experience preferred.
